Key
Responsibilities:
Client Intake Management:
Oversee the client intake process, including responding to inquiries, conducting initial assessments, and coordinating consultations.
Ensure a seamless and positive experience for potential clients.
Administrative Support:
Perform various administrative duties such as managing client records, preparing documentation, and maintaining accurate and up-to-date client information in our database systems.
Collaboration with Healthcare Teams:
Work closely with healthcare professionals to understand client needs and coordinate appropriate services.
Ensure clear communication and effective handoff of client information.
Referral Management:
Establish and maintain relationships with referral sources.
Follow up on referrals and track outcomes to support business development.
Eligibility and Benefits Verification:
Verify client eligibility and benefits for services.
Assist clients and their families in understanding the scope of services covered.
Client Follow-Up and Retention:
Conduct follow-ups with clients to ensure satisfaction with services and address any concerns.
Participate in initiatives aimed at client retention.
Reporting and Compliance:
Ensure compliance with all relevant regulations and standards.
Prepare and submit reports as required.
Qualifications:
Minimum of 2 years' experience in an intake coordinator or similar role, preferably in the home health care sector.
Strong administrative and organizational skills.
Excellent communication and interpersonal abilities.
Proficient in using healthcare databases and office software.
Knowledge of healthcare services and DDD regulatory compliance.
Ability to handle sensitive information confidentially.
Empathetic and patient-centered approach.
